Jack D. Van Slooten, age 80, of Holland died Sunday, July 14, 2019 at Medilodge of Holland. A memorial service will be 2:30 pm, Friday, July 19, 2019 at the chapel of Central Wesleyan Church, 446 West 40th Street in Holland, use entrance G. Arrangements are by Dykstra Funeral Home – Mulder Chapel.
