LANSING, MI (WHTC-AM/FM) – A subcontractor involved in the Holland Civic Center renovation accident that killed one worker and injured another is facing a six-figure fine.
The Michigan Occupational Safety and Health Administration on Thursday docked X-Treme Demolition a total of 101 thousand dollars for violations stemming from the August 1st incident in which 38-year-old Ruben Gomez-Vazques and 41-year-old Rafael Hernandez-Alvarez, both from Grand Rapids, fell from the second floor when it collapsed underneath them. Gomez-Vazques died at the scene; Hernandez-Alvarez was treated for serious injuries at Holland Hospital.
State inspectors concluded that the company repeatedly violated daily inspections for detecting hazards, had deficiencies in its accident-prevention program, didn’t have fall protection in compliance with MiOSHA’s Construction Safety Standard Rules, and for other various violations.
X-Treme was retained by general contractor G.O. Construction, who is handling the multi-million-dollar renovation of the Civic Center building and property. There was no immediate comment from representatives of the subcontractor or of the general contractor after this MiOSHA ruling.
